Bulk jobs

Ultra and Max can submit many inputs for one endpoint in a single request (if the endpoint supports bulk).

The response contains jobId, status (COMPLETED / FAILED), per-item items[] and a pollUrl. Fetch it again later with GET /v1/bulk/{jobId} (kept for 7 days). Each item costs one request; failed 5xx items are refunded. Re-sending the same Idempotency-Key returns the same job.
